Notarized Parental Consent
Florida law requires a written, notarized parental consent before a minor can receive a needle piercing. If your child is under 16, a parent or legal guardian must also be present on the day of the appointment.

Fill it out
Complete every field for both the parent/guardian and the minor. Leave the signature line blank until you're in front of the notary.

Sign in front of a notary
Bring a valid photo ID. Banks, UPS Stores, AAA branches, and many shipping/postal stores offer notary services — most charge $5–$15.

Bring it to your appointment
Bring the original notarized form (not a copy or photo) on the day of the piercing, along with your photo ID and the minor's ID or birth certificate.

- ✓Proof of relationship if last names differ (birth certificate or court order)

Where to get it notarized
Notaries are widely available. Common options:
Florida accepts remote online notarization (RON). Confirm the notary is commissioned in Florida.
Hand-push (instrument) piercing — ages 5+
Does not require a notarized form. Only a parent/guardian signature on our standard consent form on the day of the appointment is needed.
Piercer discretion
Needle piercing of minors is performed at the discretion of the piercer. If your child can't sit still safely, we may recommend the hand-push method instead, or ask you to reschedule.

Can I email or text a photo of the form?+
No. Florida requires the original notarized document to be presented at the time of the procedure.
Can a step-parent or grandparent sign?+
Only a parent listed on the birth certificate or a court-appointed legal guardian (with documentation) can sign.
Does the notary need to see the minor too?+
The notary only verifies the parent/guardian's identity and signature. The minor doesn't need to be present at notarization.
How long is the notarization valid?+
We accept consent forms notarized within the last 6 months. After that, you'll need a fresh notarization.
