Your piercing will be performed by a medically trained nurse in a safe, clean, and controlled environment. This form takes about 5 minutes. You'll need a photo of your government-issued ID, and the date of your most recent DTaP/Tdap vaccine (must be at least 14 days ago).
For minors: a parent or legal guardian must complete this form.
