Who we are
Peekaboo Piercing is a Florida-based body art studio where piercings are performed by medically trained nurses in a clean, controlled, safe environment. Your safety and comfort are our top priorities. This Privacy Notice explains how we handle the information you share with us when you fill out our consent and intake forms.
Health-information disclosure: The health information you provide is used solely for piercing safety screening. Peekaboo Piercing is not a healthcare provider and is not subject to HIPAA. See the section below for full details on how we protect your information.
HIPAA does not apply to us
Peekaboo Piercing is a body art studio, not a healthcare provider. We do not diagnose, treat, bill insurance, or transmit health information for healthcare purposes. HIPAA applies only to "covered entities" (doctors, hospitals, insurers, and health- information clearinghouses) and their business associates. A piercing studio collecting health-screening information for its own safety decisions is not a covered entity. Therefore HIPAA — and the Business Associate Agreement requirement that comes with it — does not apply.
We still treat your information with care and follow strong commercial-grade security practices described below.
What we collect
- Identity details: name, date of birth, address, email, phone
- Emergency contact information
- A photo of your government-issued ID
- For minors: parent/guardian information, ID, and signature
- Health screening answers (allergies, medications, conditions, immunization date)
- Piercing details and your signature
- Technical metadata (IP address, timestamp, browser) for audit purposes
How we use it
We use your information only to: (1) decide whether we can safely perform the piercing, (2) maintain a signed record of your consent, (3) follow up with aftercare instructions, and (4) comply with Florida Department of Health record-keeping recommendations.
How we protect it
- Encrypted in transit (HTTPS/TLS) and at rest
- Access limited to studio staff with individual logins
- Full audit log of every view, edit, and download
- ID photos and signed PDFs stored in a private, access-controlled file storage system
How long we keep it
We retain signed consent records for 7 years, in line with Florida Department of Health record-keeping guidance for body art establishments. After that, records are securely deleted.
Florida FIPA breach commitment
In the unlikely event of a security incident affecting your personal information, we will notify you in accordance with the Florida Information Protection Act (FIPA), generally within 30 days of discovery.
Your choices
You may request a copy of your record or ask us to delete it (to the extent allowed by our record-keeping obligations) by emailing the studio directly.
